Understanding Nail Excision Surgery

This term covers procedures involving the partial or complete removal of the nail plate, and sometimes modification of the nail matrix (the tissue the nail grows from) or surrounding tissues. Common reasons include:

  • Ingrown Toenail (Onychocryptosis): The most frequent indication. Surgery aims to remove the embedded nail edge and often involves procedures to prevent recurrence.
  • Nail Biopsy: Removing a small piece of the nail plate, nail bed, or matrix to diagnose conditions like fungal infections, inflammatory diseases (psoriasis, lichen planus), or suspected tumours (benign or malignant).

The Nail Surgery Procedure (Example: Ingrown Toenail)

The local anaesthetic injection stings briefly, but the surgery itself is painless. Post-operative pain is usually manageable with elevation and pain medication for the first few days.
You’ll need to limit activity and keep the area clean and dressed for typically 1-3 weeks, depending on the extent of the surgery. Complete healing takes longer. You can usually walk immediately, but rest and elevation are advised initially.
After partial or total nail avulsion without matrixectomy, the nail will usually grow back, which takes several months (longer for toenails). If a matrixectomy is performed on a portion of the matrix, that part of the nail will not regrow, effectively narrowing the nail permanently to prevent recurrence of ingrowth.
This is a common technique used after removing an ingrown nail edge. Phenol, a chemical agent, is applied to the nail matrix in that specific corner to destroy it and prevent that section of nail from regrowing, thus preventing recurrence of the ingrown nail.
Surgery is often recommended for recurrent ingrown toenails, severely painful or infected ingrown nails, or when conservative measures (like proper trimming, soaking, cotton wisp packing) have failed.
