Dandruff and dry scalp are two of the most commonly confused scalp conditions — and treating one with the other’s remedy will either do nothing or make things worse. This distinction matters clinically, and it is one we clarify regularly at our clinic.
What Dandruff Actually Is
Dandruff — seborrhoeic dermatitis of the scalp — is caused by an overgrowth of Malassezia, a yeast that is naturally present on the scalp. In susceptible individuals, this yeast triggers an inflammatory response that accelerates skin cell turnover, producing the oily, yellowish, adherent flakes characteristic of true dandruff. The scalp is typically oily, not dry. There is often associated redness and itching. Dandruff is more common in humid climates and flares with stress, hormonal changes, and certain dietary patterns.
What Dry Scalp Is
Dry scalp is exactly what it sounds like — a lack of adequate moisture and lipids in the scalp skin. It produces small, white, non-adherent flakes that shed easily. The scalp feels tight and sometimes itchy. It is more common in winter, in air-conditioned environments, and in individuals who overwash or use harsh shampoos that strip the scalp’s natural sebum.
How to Tell Them Apart
Dandruff flakes are larger, yellowish, oily, and tend to clump. The scalp is greasy. Dry scalp flakes are smaller, white, dry, and fall freely. The scalp feels tight and parched. Dandruff often comes with scalp redness; dry scalp usually does not. If your scalp is oily but flaky — dandruff. If your scalp is tight and flaky after washing — dry scalp.
Treatment
Dandruff requires antifungal treatment: ketoconazole, zinc pyrithione, selenium sulphide, or ciclopirox shampoos used regularly. For persistent or severe seborrhoeic dermatitis, topical steroid lotions and oral antifungals may be required. Dry scalp responds to gentler, sulphate-free shampoos, scalp oils with ceramides or squalane, and reducing wash frequency. Using an anti-dandruff shampoo on a dry scalp will worsen it — the stripping action increases dryness. Getting the diagnosis right first is the critical step.
