When patients present with forehead concerns, the default assumption — their own and sometimes their injector’s — is that botulinum toxin is the solution. For dynamic forehead lines (lines that form with expression and disappear at rest), botox is indeed the appropriate treatment. But a significant proportion of patients have forehead concerns that botox alone cannot address — and in some cases, botox can worsen — because the primary issue is volume loss, not muscle hyperactivity.
The Forehead Volume Loss Problem
The forehead is not simply a flat bony surface covered by skin. The supraperiosteal fat pad of the forehead — subcutaneous fat that creates the smooth, slightly convex contour of a youthful forehead — deflates with age, causing the skin to lie more directly against the underlying bone. This creates a flattened, hollowed forehead with visible temporal hollowing at the sides, and makes static lines (lines present even at rest) more prominent because the skin has lost its structural support. In these patients, applying botox relaxes the frontalis muscle but does nothing to restore the lost volume — the lines may even become more visible as the forehead flattens further without the lifting action of the frontalis.
Who Needs Forehead Filler
Patients with a hollowed, flat forehead contour with prominent static lines, temporal hollowing extending into the upper forehead, or a bossing (brow bone) that has become more prominent due to volume loss above it are potential candidates for forehead filler. The assessment involves palpating the forehead fat pad and evaluating the contour in profile — if there is visible concavity or loss of the gentle convexity of a youthful forehead, volume replacement may produce a better result than botox alone.
The Procedure and Cautions
Forehead filler is placed in the supraperiosteal plane using a cannula to reduce vascular risk. Small volumes — 0.5-1ml across the forehead — produce meaningful contour restoration. The supratrochlear and supraorbital arteries run in this region and the procedure must be performed with thorough anatomical knowledge. Forehead filler and botox are frequently combined: filler to restore volume and smooth static lines, botox to address dynamic lines and prevent new crease formation from continued muscle activity.
